Notes
The Pharmacy Department has implemented a new dosing protocol for the aminoglycoside antibiotics for some patients. This protocol utilizes a single random level 6-14 hours after a dose instead of the traditional peak and trough levels. While peak and trough levels will still be ordered on some patients, the new protocol utilizing random levels will become standard. The pharmacokinetic dosing service will write orders stating exactly which test is to be ordered. Random levels will not have a therapeutic range associated with them since their interpretation depends on the interval since the last dose.
